Autumn Grove Care Center....Where Love and Care Go Hand in Hand

    Autumn Grove Care Center is a 103 bed skilled nursing facility located in Northern Butler County.  The facility is fully licensed by the state of Pennsylvania, and is Medicare/Medicaid/HMO approved.  The Administrator and Assistant Administrator have also obtained their Nursing Home Administrator's license from the state.

The staff and management of Autumn Grove Care Center believe that each resident is an individual.  Each person should be able to live with dignity while maintaining his or her ability to function as independently as possible.

    Autumn Grove Care Center offers an "Open Inclusion Environment" that allows residents with dementia to participate in activities and enjoy all the services offered while still securing their safety and providing needed structure and support. 

    We uphold a high quality of care by maintaining high staffing levels, ongoing training, continual availability of management staff, individualized services and care plans, and much, much more.

We accept long-term stays, short-term stays and respite stay residents.
